TRANSFORMATION
The Pickaway County Family and Children First Council received a Transformation Grant through Access to Better Care (ABC) funding. Transformation funding provides:

- an intensive home-based case manager through Scioto Paint Valley Mental Health Center.
- four professionally-facilitated focus groups of:
• parents of children with disabilities
• parents of children in the juvenile court system
• parents of children who are talented and gifted and
• grandparents raising grandchildren
- the development of a pool of trained respite care providers for children with special needs

BLUEPRINT
The Pickaway County Family and Children First Council was also able to receive a Blueprint Grant
through Access to Better Care (ABC) funding. This grant allows us to place intensive schoolbased
case managers in Everts Middle School (Circleville) and McDowell Exchange School
(Logan Elm). Both of these schools have also created Care Teams.

WHAT IS A CARE TEAM?
A Care Team is a partnership of school, family, and community agencies designed to help students achieve more success in school and life. The Care Team is a group of individuals committed to minimizing students’ non-academic barriers to learning.  With Care Team, failure is NOT an option.

WHAT DOES A CARE TEAM DO?
A Care Team meets weekly to identify students in most need of assistance and to develop plans to better understand the core issues that are keeping students from being successful in school. This Team targets services for those students through a plan that is specifically designed for each student. The Team follows each student’s progress and monitors success, making changes in the plan as needed to promote achievement for that student in school and life.
