                                T.E.A.M. For Youth

                                Established by Pickaway Family and Children First Council
                                To help families with multi-need children
                                Receive adequate and appropriate services

                                HOW IT WORKS
                                • A parent, community agency, or school recognizes that a child/youth could benefit from the coordinated efforts of an individualized TEAM
                                • A referral is made to the TEAM Coordinator
                                • The TEAM Coordinator meets with the parents who will help select the members of the TEAM
                                • The TEAM is made up of the parents, and others who play a role in the life of the child/youth or persons who could provide new supports or interventions
                                • The selected members of the TEAM, including the parents, attend a meeting where a written action plan is developed and attainable goals are set.
                                • The plan includes timelines for when the actions should take place and who is responsible
                                • There is follow-up to determine if the plan is working or if it needs to be changed
                                • Limited funding is available for activities in the plan
